Narrative

The injured employee was standing on the outby corner of the #4 entry at XC-58 on the MMU 001-0 working section. While waiting on the continuous mining machine to be repaired, the injured was struck by a falling rock. The rock fell from the mine roof which measured approximately 8.5 feet high. The rock measured ~ 4' x 2' x 0-4"" thick.
