Narrative

Mechanic was inflating tire on 988F cat wheel loader when tire failed, causing sudden release of air that had been under pressure. 3 miners in area were injured w/flying debris. Later determined that the acc. did not meet requirements for immediate report. Pursuant to MSHA's guidance, this report is being modified to reflect that this did meet requirements of immed. reportable acc.
