Narrative

Employee states he was in the process of pulling the bit sleeve off a miner. The sleeve was tight on the miner, so employee used the end of a chisel to shim the ram, in order to push the sleeve out. As he removed his hand from the chisel, the chisel shot out and punctured employee's right hand.
