## Narrative

EE was climbing up a ladder to remove a copper line to clean a fire eye.  As he got to the top of the ladder it started to slide off its resting place sending him to the ground.  He fell on his hip and dislocated his left shoulder.  He did not get a chance to tie off because he did not get to the top of the ladder.  Ladder was a 22 ft extension ladder.
