## Narrative

On 11/21/11 employee was hanging a block curtain in #2 entry when the drill opr. turned the cable reel on. His feet were in a loop of cable and swept him off his feet. He stated that he felt pain in his left shoulder. He had surgery on his shoulder on 4/3/12. We obtained confirmation on 4/3/12 that this was related to the injury that occurred on 11/21/11.
