## Narrative

EE was performing a normal pre shift inspection on the cone crusher. He checked the oil and stepped up onto the belt guard to inspect the cone. When he attempted to dismount the guard his foot slipped, he was unable to hold his weight with his hands and he fell on to the floor and handrail, bumping his head either on top/bottom rail. He was diagnosed with concussion.
