## Narrative

Employee was assisting in cleaning the inside of #3 rail scale.  The manhole was uncovered.  Employee picked-up the manhole cover to put it on the manhole.  The cover weighed approximately 50 pounds.  The manhole started slipping out of his hands.  EE tried to catch the cover, when he did, he fell down and he felt a sharp pain in his right arm/shoulder area.
