## Narrative

Employee was injured 12/15/11 when he fell off motor grader. He has been on medication to relieve the pain but has not found relief. His first shift missed was 3/17/12. On 4/9/12 after completion of a workers compensation investigation, Westmoreland Resources was informed that a herniated disc in employees back was the result of the on the job injury.
