## Narrative

Non-compliance of company & mine site safety policies in regards to equip. & PPE. EE while wearing safety glasses, did not don face shield while using an elec. hand grinder that he had remove the handle from. While prepping a weld, 1/8"" abrasive grinding wheel got caught between pipe openings, kicked back into EE's face, causing 1' cut to his face just outside of his left eye.
