## Narrative

Mining a sixty to the right, while cutting the roof, the miner bits caught a bacon strap taking out a couple roof bolts allowing the bacon strap to be thrown back towards the miner operator striking him at the bridge of the nose and above the eye. PPE was not a factor, compliance rules/regs was not a factor, mining equipment/system was a factor, miner knowledge was a factor.
