## Narrative

EE was washing equipment at the lube shop wash bay. As he was walking backwards he slipped on some mud resulting in him twisting his right ankle. EE initially refused medical treatment. On 4/13/2012 EE requested to be taken into clinic. EE was placed on restricted and released back to work.
