## Narrative

Employee was operating a UG Haul Truck on FWL, when he drove through a hole that had some water in it and hit a rock, causing his body to shift upward, and he struck his head on the inside of the cab on the haul truck. He reported neck pain. We took him to medic on site & down to ER. Doctor issued restrictions.
