## Narrative

While the employee was mixing mud on top of the mud deck, he removed a bucket of Quik Trol that was covering the bung access hole in the grating of the mud tank. The employee moved the empty bucket out of the way; this removed the cover on the access hole.  The helper mixed mud when his right foot slipped into the access hole.  He then lost his balance and twisted his ankle.
