The tailpiece has a lot of rock that had fell off belt due to narrow belt and rib rolls - lot of water, causing employee to slip and sprain his right foot. Employee did not go for treatment until 5/17/2012 and it was determined that he had a hairline fracture. EMPLOYEE STARTED LOSING TIME 5/22/2012.
