## Narrative

Employee was helping move MRS, he was 40' back watching the cable and the MRS operator was 25' away from MRS when he pressurized it against the mine roof.  The bolt end broke off and became like a projectile striking employee on the lenses of his left eye glasses breaking the left lenses and striking the left eye.
