## Narrative

Employee was using a 5 foot crowbar to maneuver the eye of a dump cylinder which was hoisted on an overhead crane. The employee stood on an elevated platform 22 inches off of the ground as he applied force. He stumbled off of the platform when the eye moved, placing his left hand down to break the fall resulting in a possible fracture to the left radius.
