Narrative

While installing the drive coupling on new bearing unit, the taper lock bushing was spread with a small steel wedge. After positioning the bushing on the shaft, the employee started to tap the wedge back and forth in order to remove the wedge. With the first tap, the wedge flew out striking the employee in his upper lip area.
