## Narrative

Employee was dismounting LHD Number 11 when he slipped because of a dusty hand rail and steps. Employee struck head and back when he slipped.  Employee refused medical treatment at first.  After, he decided to go to the doctor because of the bruises.  Doctor placed employee off work on June 11, 2012.
