Narrative

On 5/17/2012 employee was using a slide hammer when he repositioned at 40 degree angle to get a better position he felt a pop & burn in Right shoulder, treatment as first aid. 4 weeks later employee reported that while working on small tire shoulder began to hurt again. Medical treatment was ordered on 6/21/2012.
