## Narrative

While grinding a burr off of a grader blade, a piece of metal apparently got into his eye. The metal had to be removed by an opthomologist. This accident was reported on the MSHA 7000-2 quarterly report but an internal audit indicated that the 7000-1 had not yet been submitted.
