The employee was trying to retrieve his drill steel from the roof. When he reached for the steel it sprang loose striking his right hand between the thumb and index finger causing a laceration that required stitches. This accident was reported on the MSHA 7000-2 quarterly report but an internal audit indicated that the 7000-1 had not yet been sent.
