## Narrative

The skip contacted the 1200 Level landing and caused the hoistman to shut the hoist down and dispatch personnel to investigate. The skip was twisted in the compartment as the bottom 100' of west guides had broken free and fallen down the shaft. The guide bracket failures were likely due to an incident that occurred the previous Friday, but the damage was not visible when inspected.
