## Narrative

Unloading tailgate drive with a Petito mule went to adjust canopy and jack supporting it failed causing the canopy to drop striking his head, neck, and shoulders. PPE was not a factor, compliance rules/regs was not a factor, mining/equipment systems was a factor, miner proficiency was not a factor.
