Narrative

The injured was operating #96 wallace mantrip, the vehicle hit a bump in the road way causing the injured to strike his head on the canopy. The injured was treated for a neck strain and released without restrictions. On 7/13/2012 he was taken off work due to headaches and diagnosed with occipital neuritis.
