## Narrative

Employee saw a rock that he thought was loose. He tried to pry it down, then decided to spot bolt it up. As he was drilling, the rock broke and fell, landed on the protective canopy over his head, and then bounced off, striking the employee on his left shoulder. He fell down and injured his left knee also. The area had been bolted and meshed prior to the incident.
