## Narrative

The employee was installing a roof bolt from the right side of the continuous mining machine. Employee was installing an additional bolt in the roof strap at an angle when the bolt wrench and the roof bolt head broke, causing the bolt wrench to strike the employee in the face below his nose. Employee suffered facial fractures and cuts requiring sutures.
