## Narrative

While getting into a mantrip in June 2012 jammed his head and neck into a 6"" waterline and strained his lower back. The impact knocked him to the ground and dazed him. He did not report the incident at the time and sought medical assistance 7/17/2012 where he was diagnosed with a Lumbar and Cervical strain and began working under a restriction.
