Narrative

Employee was working as a roof bolter operator opposite the operator's side. The operator was moving the bolter to the next row of bolts to be installed. He hesitated and the victim stepped up to the bolter to get a glove. The operator move the bolter a few more inches and the bolter tire got on the victims toes. The big toe of the right foot was fractured.
