## Narrative

Employee was conducting maintenance work with three others on a shovel. They were removing the upper rails on the shovel using an impact gun and wood skids. Later in the shift, the employee felt pain in his lower back. The employee did not report the pain until 8/2/12.  He went to the hospital on his own and they found a disc diffusion in L4-L5.
