Narrative

The Roof Bolter operator had installed his first bolt in a face. As he was preparing his second bolt some draw slate fell inby the ATRS in the unbolted area. Apparently a piece of the slate flipped back and struck him on the left leg and hip. He returned to work the next day but started missing on 8/21/12 due to continued pain in the hip area.
