## Narrative

EE was cleaning Prep Plant floors and steps, started down a set of steps while cleaning with water, the third step from the bottom had a bend in the step on one side. He slipped on it, fell on left side. PPE was not a factor, mining equipment/system was not a factor, compliance rules/regs was not a factor, miner proficiency was a factor. Became a lost time injury on 9/13 2012.
