Narrative

Employee was using an adjustable wrench to loosen a 3/4"" bolt. The bolt came loose suddenly, allowing the wrench to strike the employee's right thumb and wrist. No broken skin or bruising, but employee was seen by physician and placed on restricted duty-no lifting over 15lb w/ right hand
