Narrative

On February 15, 2012 the Employee bent a roof bolt which caused pain in his shoulder. The Employee continued working until October when his shoulder began to swell. The Employee was seen by a physician who ordered a MRI. The MRI Showed mild tears in the tendons in his shoulder. The Employee was taken off work on October 11,2012
