## Narrative

Operating Scoop Tractor - Pulling belt with scoop, when the belt caught in the rack, the belt grab slipped off the belt causing the scoop to lunge forward causing the employee to strike the back of his head on a supertwist bolt causing a laceration requiring sutures. Scoop is not equipped with a canopy due to the low mine height of 37-inches.
