## Narrative

Employee was replacing cutter head on continuous miner and was attempting to replace top pin of the cutter jack. Coworker raised jack with slate bar in upward motion. Employee was using his hands to pull jack in place to replace pin. The slate bar slipped allowing cutter jack to smash Employee's left ring finger against frame of pan.
