## Narrative

Employee stated he was taking a hubcap off a Komatsu 630E and slipped and fell to the ground injuring his left hip and right knee. Employee's family physician referred him to St. Marys Occupational Health Center. Dr. diagnosed him with a sciatic nerve issue on his left hip and sprained right knee and took him off work.
