## Narrative

Employee was pulling on a pipe wrench trying to rotate a shaft on a screw. The shaft was locked out and tagged. The employee was using excessive effort that caused the shaft twist like a torsion bar. The employee lost his grip on the wrench and stored energy in the twisted shaft caused the wrench to hit the employee's right hand breaking a bone.
