Narrative

Employee came in to work 9/6/12 with a doctor's slip stating employee to remain out of work until 12/12/12. Employee was alleging bulging disk in neck, back, shoulder, and problems with hips and knees due to repetitive motion. On 11/20/12 received paperwork from workers' comp. board stating from a 11/6/12 hearing the board awarded EE temporary total disability.
