Narrative

Miner was operating loader Underground on 11/29/2012 he told the supervisor he wanted to go home two hours into his shift when asked if he was sick or hurt he replied ""No"". The next day the employee (Miner) returned to work with a Doctors note saying he had a Strain, Neck Spinal Cord injury.
