Injured complained of right shoulder pain and saw a physician on this date. X-rays were taken and he returned to work. He continued to work and subsequently felt pain in his shoulder. An MRI was done on 6/14/12 indicating degeneration. He again returned to work until 12/03/12. He then went off work for surgery.
