## Narrative

Employee stepped up approx. 10"" onto the take-up bolt for the feed belt. When he lifted his other foot off the ground to climb up onto the belt, the foot on the bolt slipped and he fell backward onto the ground, striking his right arm. No immediate injury noticed, but when medically evaluated, a bone chip to the right forearm was discovered.
