The employee was carrying supplies on the longwall tear down face, he was walking in the pan line ( Locked and Tagged out). He hit his head on a shield that was lower than the others, which knocked him backward and he fell. The employee did not seek medical treatment or report this until 12/22/2012. He has a Cervical Sprain.
