Narrative

On E4 Longwall setup face, the canopy of shield 72 was under 73 side shield a T-Board was placed between the shields when pressurized the T-Board shattered in pieces when shield kicked into place and a piece of the wood struck employee in the mouth causing laceration to lip and broken tooth.
