## Narrative

Installing tooth on LD995 corner adapter, tooth slipped off and dropped approximately 4"", injuring left middle and ring finger. Diagnosed with small fracture to fingertip, one fingernail removed by doctor. Fingers were bandaged and employee was released on light duty.
