Narrative

EE was standing on a 4' ladder to set a spool when he slipped & fell to the ground. EE struck his left elbow on the bottom crushing & bruise felt. EE was holding a metal roof strap & when he fell he received a small 3/8"" laceration to the left index finger at the first joint. Bleeding was very minimal. EE exited the mine under his own power and refused treatment and/or ambulance.
