Narrative

EE working with bleeder crew standing a safe distance away from bleeder being lifted by crane. When pipe weld broke the EE moved to avoid a possible pipe swing; slipped on mud and fell on Tailings pipe. EE was checked that day; on February 5, 2013 EE was taken to medical facility for complications due to slip and fall. Prescribed narcotics for pain; lost time on February 5, 2013.
