Narrative

EE moving regular screens(non-ceramic)in kneeling position, he turned to toss a screen down; felt pull or strain in left shoulder. Late reporting of incident; not reported to supervisor until February 1, 2013. EE continued to work. On February 8, 2013 EE was seen by Specialist and put on restricted duty for use of shoulder.
