Narrative

Irregular surface with water filled holes, damp conditions, EE was walking the roadway in entry #6 section ""A"" when he fell to his knees in a water filled hole, he felt a severe pain in left knee. He worked until 4:00pm. EE was seen at the ER of Broaddus Hospital in Phillippl, WV at 9:00pm 3/4/13. DX is pending.
