## Narrative

Injured worker swept off concrete pour form with his left hand. A co-worker was busting up dried concrete in close proximity. The injured worker inadvertently placed his hand in the path of the shop hammer, and his left middle finger made contact with the hammer. The individual received medical treatment requiring stitches.
