Narrative

Employee was walking and the grating tipped resulting in the employee falling to the ground. Initial medical evaluation resulted in the employee returning to full capacity work. During a routine occupational medicine follow up on 4/17/2013, the employee stated that he was still in pain and medical evaluation revealed the employee will be out of work to allow time to heal.
